CCI Physician Locator & Prescription History Report:  A New Discovery Tool!

CCI is able to offer a new discovery tool which provides clients with a detailed five-year list of physician prescriber and prescription history information regarding a specific patient.*

The CCI Physician Locator & Prescription History Report includes:

• Prescribing physician’s name, address, phone number and specialty
• Address & phone number of pharmacy where prescriptions were filled
• Drug name and dosage
• Date that the prescription was filled and the number of refills
• Drug detail (e.g., disease usage information, therapeutic class and side effects)
• A report outlining locations from which records may be requested and an opportunity to order those record

What is required to obtain the report?

• A specific HIPAA-compliant authorization
• Full name of patient (if possible, please include maiden name and/or other AKAs)
• Gender
• Date of birth
• Social Security Number
• The ZIP code of the patient’s current residence

Turnaround time:

The CCI Physician Locator & Prescription History Report will be produced within 48 hours following receipt of a valid patient authorization. The report is also available on a rush basis.

Source of Data:

Information contained in the CCI Physician Locator & Prescription History Report is derived from the nation’s Pharmacy Benefit Managers. *Because the data is voluntary and some insurers and pharmacies currently do not participate, the database covers approximately 75-80% of residents in the United States. Also, if the patient did not utilize a prescription plan to obtain the drug, certain information may not be included.
