Prescription History Report
CCI is proud to announce the launch of a new discovery tool that provides clients with a detailed five-year list of physician prescriber and prescription history information regarding a specific patient.* Click here to see a sample report.
The CCI Physician Locator & Prescription History Report includes:- Prescribing physician’s name, address, phone number and specialty
- Address and telephone number of pharmacy where prescriptions were filled
- Drug name and dosage
- Date that the prescription was filled and the number of refills
- Drug detail (e.g., disease usage information, therapeutic class and side effects)
- A report outlining medical providers from which CCI has not requested records and an opportunity to order those records
What is required to obtain the report? Order Form
- CCI HIPAA-compliant Rx authorization form
- This is the ONLY authorization that can be accepted for a CCI Physician Locator & Prescription History Report
- Full name of patient (if possible, please include maiden name and/or other AKAs)
- Gender
- Date of birth
- Social Security Number
- The ZIP code of the patient’s current residence
- The CCI Physician Locator & Prescription History Report will be produced within 72 hours following receipt of a valid patient authorization.
- The report is also available on a rush basis (24 hours) for an additional fee of $25.00.
- Information contained in the CCI Physician Locator & Prescription History Report is derived from the nation’s Pharmacy Benefit Managers.
